Who is eligible for this plate? Any owner of a motor vehicle who is certified by the Kentucky chapter of the Pearl Harbor Survivors Association Relevant KRS: KRS186.041 Steps Required: Submit proof from the Kentucky Pearl Harbor Survivors Association to the County Clerk. Initial Cost and Cost each time a new plate is issued: $20.00 Annual Renewal Cost: $3.00 Decal Expires: Customer's Birth Month Remarks: This plate may be issued to any 6,000 lb. commercial or passenger vehicle owned, leased, or provided by an employer. The initial cost includes $5.00 which goes to the Veterans Trust Fund. A minimum of $5.00 may be donated to the Veterans Trust Fund at the time of renewal. This type of plate is issued every five years, and will be replaced in the year 2003 and every 5th year after.
