Who is eligible for this plate? Any owner of a motor vehicle, required to be registered for Kentucky highways, who is a recipient of a Distinguished Service Cross Award. Relevant KRS: KRS186.041 Steps Required: Submit an application to the Kentucky Department of Veterans' Affairs along with a copy of the general order that authorized the award and the recipient's Department of Defense form 214. To personalize, this plate, submit an application to the county clerk by September 1st of the year preceding the registration year. Initial Cost and Cost each time a new plate is issued: $3.00 Annual Renewal Cost: $3.00 Decal Expires: Customer's Birth Month Remarks: If the vehicle has a "regular" registration plate, that plate must be turned in to the County Clerk. Credit will be given for any months remaining on the "regular" registration. The personalized plate cannot conflict with the state number/letter system nor be offensive to good taste and decency. The customer has the option of ordering a "new" plate each year or renewing with a decal only. $5.00 of the initial fee goes to the veteran's trust fund and at renewal time, the individual has the option of donating a minimum of $5.00 to the fund. Maximum number of characters for this plate is five (5).
